بیان مسئله

کودکان مبتلا به کاهش دید یک چشم به ندرت از ضعف بینایی شکایت می کنند و بنابراین مشکل آنها تا مدتهای مدید ناشناخته می ماند و اگر دچار انحراف چشم واضحی نباشند مورد غفلت خانواده قرار می گیرند(17). چنانچه اختلال بینایی کودک درمان نشود در روند یادگیری کودک ایجاد اختلال می نماید(5). غربالگری حدت بینایی[1] روش مقبول شناسایی اختلالات بینایی در سنین کودکی است(18). هدف از غربالگری بینایی تشخیص و به دنبال آن درمان به موقع عیوب بینایی که می توانند تنبلی چشم را به دنبال داشته باشند(پیشگیری سطح اول) و شناسایی زودرس و درمان تنبلی چشم(پیشگیری سطح دوم) است. در کشور ایران برنامه غربالگری کودکان پیش دبستانی، يکی از برنامه های جاری نظام سلامت است و طبق دستورالعمل سازمان بهزیستی از سال 1375 همه ساله در ماه آبان انجام می شود. بنا برگزارش سال 1390 مرکز توسعه پیشگیری از معلولیت های سازمان بهزیستی کشور، با اجرای این برنامه سالانه حدود دو میلیون کودک تحت پوشش مهدها و آمادگی های سطح کشور، توسط مربیان مورد سنجش بینایی قرار می گیرند(19). در اين برنامه غربالگری، سه سطح ارجاع در نظر گرفته شده است. به اين ترتيب که معاینه اولیه را مربیان آموزش دیده مهدهای کودک انجام می دهند، موارد مشکوک به اختلال بینایی از مربیان به کارشناسان بینایی سنجی ارجاع می شوند تا مورد بررسی های تشخیصی قرار گیرند. چنانچه در سطح دوم، انجام معاینات تخصصی تر لازم دانسته شود کودک به سطح سوم یعنی چشم پزشک ارجاع و مورد بررسی بیشتر قرار می گیرد(20) و پايش خدمات تا اين مرحله از ارائه خدمت بر اساس برنامه ای مدون انجام می شود. در برنامه غربالگری تنبلی چشم و اختلالات بينایی مسئوليت مداخله و پی گيری مداخلهبه کارشناسان بينایی سنجی و چشم پزشکان سپرده شده (19) و پروتکل واحد و مشخصی برای ارزيابی فرآیند توانبخشی تدوین نشده است. بی شک در این برنامه توانبخشی نيز می بايست همانند ساير برنامه های نظام سلامت برنامه ای برای ارزيابی فرايند اجرای توانبخشی تدوين گردد و ارائه خدمت و نتايج حاصله بطور منظم مورد ارزيابی قرار گيرد تا بتوان از انجام درست و بموقع توصيه ها و دريافت به موقع خدمات اطمينان حاصل نمود و چنانچه عواملی در انجام برنامه خلل ايجاد می کنند، برای رفع به موقع آنها چاره انديشی کرد. بنابراین در صورتی می توانيم انتظار موفقيت برنامه را داشته باشيم که مطمئن شويم پس از تعیین کودکان مشکوک به ابتلاء به مشکل بينایی در سطح اول غربالگری و انجام اقدامات تشخيصی تکميلی در سطح دوم و سوم ارجاع و شناسايیآنانی که دچار مشکل بينايی هستند، مراحل درمان و توانبخشی نیز بموقع و بطور مستمر انجام می گيرد. مدير برنامه غربالگری برای نظارت بر فرآيند توانبخشی و حصول اطمينان از اينکه اجرای برنامه از کارایی و اثر بخشی برخوردار می باشد، نيازمند آن است کهاز نتايج پايش فرایند توانبخشی آگاه باشد تا مطمئن گردد، پس از تشخيص و توصيه انجام مداخلات گوناگون(الف) امکانات مورد نياز برای انجام مداخلات در دسترس خانواده ها قرار گرفته است، (ب)درمان توصيه شده مورد پذيرش کودکان و خانواده ايشان است و آنان بطور مستمر انجام مداخله را دنبال می کنند.(ج) درمانگران بطور مستمر بر اجرای برنامه توانبخشی پایش و نظارت دارند. (د) موانع انجام مداخلات بموقع شناسائی و در جهت رفع آنها تلاش می شود. و بالاخره اينکه (ه)خدمات ارائه شده تا چه حد مفيد و اثر بخش بوده اند و البته اگر در فرآیند توانبخشی بینایی خدشه ای وارد شود، از کارآیی و اثر بخشی کل برنامه غربالگری بينایی کاسته خواهد شد.

مقدمه

نظام سلامت از طريق طراحي و اجراي برنامه هاي سلامت به ماموريت خود اقدام مي كند. مديريت صحيح نظام سلامت در گروي مديريت صحيح برنامه هاي تشكيل دهنده آن است(1). بر اساس شواهد علمی موجود اگر وضعيت ارائه خدمات در نظام سلامت ارتقاء نمی يافت، در طی دو دهه اول قرن حاضر، تعداد موارد نابینایی در جهان تقریبا دو برابر می شد.برای پيشگيری از اين وضعيت بود که تلاش همگانی جهت طراحی و اجرای برنامه سلامت بینایی تا سال 2020 میلادی گسترش یافت. مغز انسان بالغ بر 75 درصد داده های محیطی را از طریق بینایی دریافت می کند، بنابراين چشم ها از مهم ترین اندام های ارسال اطلاعات حسی به مغز هستند (2و3)و امکان ارزیابی دنیای اطراف را برای آدمی فراهم می آورند (2).حس بينايیدر درک جهان نقش عمده ای دارد (4) و برقراری ارتباط با محیط اطراف را تسهیل می کند(2). در صورتی که ادراک بینایی دچار اشکال شود، عملکرد فرد در انجام فعالیت های روزمره زندگی از جمله غذاخوردن، لباس پوشیدن، رانندگی، تعاملات اجتماعی، اشتغال، خواندن و نوشتن دچار اشکال می شود(5). از اين رو، حفظ سلامت چشم ها از اهمیت خاص برخوردار است. اختلال بینایی[1] یک مشکل بهداشتی جهان است. (6) آمار گوناگون شيوع متفاوتی از اين مشکل را در بين کودکان گزارش کرده اند برخی از گزارشات حاکی از آنند که اين اختلالات 20/1 افراد قبل از سنین مدرسه را درگیر می کند(7).مطالعات دیگری نشان داده اند که 1 درصد کودکان سنین مدرسه دارای عیوب انکساری هستند(8). به طور کلی تخمین زده می شود در جهان 42 میلیون نفر دچار نابینایی و 135 میلیون نفر دیگر دچار عیوب انکساری شدید باشند(6). توانايی ديدن در تکامل طبیعی جسمانی و ذهنی و ادراک شناختی کودکان نقش مهمی دارد (4). بروز اختلالات بینایی در سالهای نخست زندگی می تواند در تکامل بخش عصبی دستگاه بینایی تداخل ايجاد نمایند و کاهش عملکردی برگشت ناپذیر بینایی که به آن تنبلی چشم (آمبلیوپی)[2] می گویند را به دنبال داشته باشد(8و9). در صورت ایجاد تنبلی چشم، کم بينايی حتی پس از اصلاح اختلال اولیه بینایی (مثلا اصلاح جراحی افتادگی پلک یا استفاده از عینک مناسب برای اصلاح عیب انکساری) باقی می ماند(10). بهترین سن شناسایی و درمان تنبلی چشم را قبل از 5 سالگی دانسته اند. با افزایش سن و تکامل سیستم عصبی بینایی، میزان پاسخ به درمان کاهش می یابد(11). شیوع تنبلی چشم در گروه سنی «پیش از دبستان» در جهان، 4-1 درصد گزارش شده (12و13)و در کشور ایران حدود 2 تا 5 درصد تخمین زده شده است(14). پس از تشخيص قطعی وجود و نوع اختلال بينایی، مرحله درمان و یا توانبخشی فرا می رسد. نقص بینایی کودکان بدون درمان و یا توانبخشی می تواند موجب بروز مشکلات آموزشی و ارتباطی شود. با انجام توانبخشی بینایی می توان از شدت مشکلات کاست و یا در برخی موارد آنها را برطرف نمود(15). مداخلات توانبخشی ممکن است بر حسب مورد شامل اصلاح عیوب انکساری، درمان طبی مشکلات چشمی «تنبلی چشم زا»[3] و در مواردی که قبلا تنبلی چشم ایجاد شده ترکيبی از یک یا چند مورد از (1)عینک برای چشم درگیر و (2) پوشاندن(اکلوژن[4]) و یا (3) آتروپین[5] موضعی برای چشم غیردرگیر باشد وهمچنیناز دیگر روش های درمانی می توان به استفاده از دستگاه کم[6]( ایجاد تحریکات بینایی در سطح کورتکس بینایی در مغز) و نرم افزارهای کامپیوتری(مانند اپتوسیس) اشاره نمود (16و42و43).
